SLPTA has allocated $300 per classroom for supplies. Supplies include decorations, food, prizes, and Teachers Pay Teachers. Not included would be volunteer gifts and items not intended for classroom use.

Instructions for submitting Reimbursement Request

1. Please fill out THIS FORM and attach receipts. If you prefer PTA to pay vendors directly, please communicate with the PTA Treasurer.

2. Email the filled out form and receipts to treasurer@summerlakepta.com.  Your reimbursement check will be issued up to 3 days after it was received. To send a hard copy, please leave in our Teacher Liaison Ms Casares' box.
